To help expedite your visit, please take the time to print out the forms below and then bring them with you on your first visit. It is important to provide your physician with a complete and accurate history.

Frequently Asked Questions

  • Driver’s license or State ID
  • Insurance information
  • Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits
  • Medications list (if any)
  • If you have had surgery elsewhere, please bring a copy of your operation report

During your initial visit, you will be given a comprehensive medical evaluation and ask you a series of questions. The doctor will then explain if he thinks you should have a specific test or procedure and any treatment options will be discussed with you at this time.

Your medical file is handled with the utmost respect for your privacy. Our staff are bound by strict confidentiality requirements as a condition of employment regarding your medical records. Ordinarily we will not release the contents of your medical file without your consent.

Depending on your insurance, you may be required to provide a referral when you come to our office. It is recommended that you call your primary care physician or your insurance company for this information or you can call our office and we will be able to help you with this answer.

Before seeing any medical specialist, it is always preferable to talk to your own family doctor, who can discuss your condition with you and advise on whether any specialist care is appropriate. If it is, he or she can help you to choose the specialist best suited to your needs. Your family doctor can help the specialist to care for you better by providing relevant information about your health. Communicating with the specialist will also enable your family doctor to care for you better during and after your specialist treatment.

Your insurance has determined if you have a copayment to see a specialist. This amount will be collected at the check in window, as well as any deductable that your insurance has set for your plan.
